New Patient Info

Before you arrive

Our office requires a referral from another physician before an appointment can be made. Once you have an appointment a new patient packet will be mailed to you which you will complete and bring with you to your appointment. We ask that you arrive at least 15-20 minutes prior to your appointment so that we have your chart and records ready for the doctor at the appointed time. We will be taking your picture for our electronic chart on your first visit, if you prefer to bring a current picture, we will be happy to scan it into your chart. This requirement is for the protection of your private health information and to avoid identity theft. We also require a copy of your insurance card and photo ID.


We are HIPAA compliant and you will be given a copy of our privacy policy. If you have any questions about our HIPAA privacy policies, contact our office 868-9244 and ask for the HIPAA compliance officer.


Be sure to bring your insurance card with you. Eugene Springfield Nephrology provides care for patients in most managed care and standard insurance plans. You should check with your insurance company if you have questions about whether we are on their panel of providers. All of our physicians accept Medicare assignment, but you will be required to pay the balance after Medicare or your supplement has paid.

Any insurance copays must be paid at the time of your visit. Payment can be made by check, cash or credit card (Visa or MasterCard). Remaining balances after insurance has paid are due within 30 days. If you are uninsured or are not able to pay your balance in full, contact our business office to work out a mutually acceptable payment plan. The business office telephone number is (541) 485-7137.
